Note that you may fill out the form and then return later to pay your fee. Your application will not be reviewed until the payment has been received. 

To CONFIRM YOEUR ENROLLMNT: 

1. If you have been offered acceptance, you may Login  to your account used to submit your application payment by clicking the link at the bottom of the page.
2. Click Start a new Application > 2026 DDS Enrollment Deposit.
3. Fill out the acceptance form with your information.
4. Finalize your application and pay the acceptance fee.
5. After your payment has been processed, you will receive an email from our office with additional information about enrolling in the fall.

1. Click Start a new Application > Dental Hygiene & Dental Therapy > 2026 Dental Hygiene/ Dental Therapy Application
2. Follow the application instructions and fill out your information. 
3. Complete the Undergraduate Admissions Transfer Application (or Change of College Form for current students).

  Applications for the Dental Hygiene/Dental Therapy program(s) are due January 5th, 2026 at 11:59 PM (CST).

All materials (transcripts. letters of recommendation, etc.) must be received no later than January 26th, 2026.
