    Doctor of Dental Surgery (DDS) Applicants

    To CONFIRM YOUR ENROLLMENT: 

    1. Login  to your account used to submit your application payment by clicking the link at the bottom of the page.
    2. Click Start a new Application > 2024 DDS Enrollment Deposit.
    3. Fill out the acceptance form with your information.
    4. Finalize your application and pay the acceptance fee.
    5. After your payment has been processed, you will receive an email from our office with additional information about enrolling in the fall. 
    To upload Documents: 
    To upload your Eye Exam and/or Technical Standards form(s):
    1. Log in  to your account used to confirm your enrollment by clicking the link at the bottom of the page.
    2. Open your 2024 DDS Enrollment Confirmation application to access your status page. 
    3. On the status page, you will be able to see what documents you still need to submit.
    4. Submit your documents by attaching a .docx or .pdf file and selecting upload.
